Book Description:
This volume presents a critical appraisal of the neurological literature on neuromuscular disease from an evidence-based perspective. Writing in a readable, accessible style, the author considers, in detail, a broad range of the published literature relevant not only to questions of treatment, but also to issues of diagnosis and prognosis. Through a series of questions and answers concerning specific neuromuscular disorders, each chapter critiques the best available evidence to illustrate strengths and weaknesses of the data and make the reader aware of the quality of clinical research studies in general. Introductory chapters facilitate this learning process by elucidating the epidemiological and biostatistical issues pertinent to diagnosis, treatment, and prognosis. A broad range of disorders of the anterior horn cell, nerve roots, peripheral nerves, neuromuscular junction, and muscle are critically appraised and discussed.

http://www.cic.gsk.com). The Centre for Neuromuscular Diseases exists to develop the understanding of neuromuscular disorders in new diagnostic techniques and practical therapies, and contains newly installed Siemens 1.5T (two) and 3T scanners and a 9.4T Varian system. GSK has partnered with Imperial College London to develop Europes largest new research Clinical Imaging Centre.
This unique partnership between academic scientists and the pharmaceutical industry has been forged to develop and implement novel strategies for applying imaging to better understand mechanisms of disease in order to accelerate the introduction of new pharmaceuticals. Complementing 2 PET/CTs, 2 cyclotrons and a substantial radiochemistry and radiobiology group, the CIC has a new Siemens 3T Tim Trio MRI scanner with plans for delivery of a second high-field system in mid-2008.
In collaboration with Imperial College, additional multi-modal, preclinical imaging facilities will become available in 2009 (MRI, PET, optical), although use of these will not be central to the currently advertised position. The CIC works closely with the Centres for Excellence in Drug Discovery (CEDDs) in GSK, which will enable access to substantial bioinformatics and clinical pharmacology support, as well as new experimental therapeutics not generally available to the outside research community.
The appointee will join an expanding community of MRI researchers at the IoN and CIC sites, and work in a rapidly growing field applying state-of-the-art imaging technology to advance both clinical science and drug discovery. The main focus of the position will be to establish a reproducible MR methodology to assess regional and total muscle mass in humans, as a means to measure response to experimental therapies. Potential research projects to which the appointee may contribute include the development and optimisation of measuring techniques; assessment of imaging and processing protocols; and both follow-up and longitudinal clinical studies.
Applicants should have, or be close to obtaining, a PhD in physics or biomedical engineering, or have equivalent research experience, with a strong knowledge of MR physics and a background in MRI pulse sequence development. Experience with C/C++ programming, Matlab, and Siemens MRI scanners and software (IDEA, ICE) is desirable. Applicants should be highly motivated and capable of working both as members of a team and independently. The post is available immediately and is funded for 3 years. Starting salary on UCL Grade 7 in the range 30,115 - 34,489 pa inclusive, superannuable.

Abstract:
Purpose of review: The aim of this article is to review the latest clinical trials in neurological diseases where magnetic resonance imaging was used to assess treatment outcome.Recent findings: The unique sensitivity of magnetic resonance imaging for detecting disorders in the brain has made it an attractive noninvasive tool for assessing treatment efficacy in several diseases. Volumetric and functional magnetic resonance imaging have proved to represent robust biomarkers for the evaluation of anti-Alzheimer treatments, and have demonstrated a significant impact of cholinesterase inhibitors. The optimization of thrombolytic therapy in acute ischemic stroke has concentrated on the quantification of the ischemic penumbra, using perfusion-weighted and diffusion-weighted imaging. Standard assessment of T2 or fluid-attenuated inversion recovery lesion load remains the method of choice to evaluate new therapeutic strategy in multiple sclerosis. Other nonconventional quantitative magnetic resonance imaging techniques such as magnetic resonance volumetry, magnetization transfer imaging, diffusion-weighted imaging, or magnetic resonance spectroscopy are increasingly used in the field.Summary: Magnetic resonance imaging has become a major surrogate marker of treatment response in clinical trials of neurological disorders, offering the possibility to reduce the required sample size or to shorten the duration of the trial.(C) 2008 Lippincott Williams Wilkins, Inc.

The Handbook of Clinical Neurology (Series Editors: Michael J. Aminoff, Department of Neurology, University of California, San Francisco, USA; François Boller, Bethesda, USA; Dick F. Swaab, Netherlands Institute for Neuroscience, Amsterdam, the Netherlands)has an international reputation as the ultimate reference tool for neurologists involved in clinical research and for neuroscience research workers. It offers comprehensive coverage of the whole field of neurology edited and written by an international list of contributors including the leading workers in the field. This volume is particularly for all those interested in the fundamental aspects of muscle disease or involved with care of patients with these disorders. However, a further separate volume in the Handbook of Clinical Neurology series will be devoted to the muscular dystrophies. The contents include coverage of the latest developments in the subject such as the regulatory nuclear and mitochondrial genetic mechanisms, the proteins involved in muscle function and their remarkable interactions, the nature and function of ion channels and ionic activity, and the complexity of inflammatory cascades that has led to a wider appreciation of the nature of muscle diseases and suggests new approaches for their management. Clinically there has been the recognition and description of new entities and improved classifications of groups of disorders such as the distal myopathies, and the discovery of disease-causing mutation for many of the muscular dystrophies and hereditary myopathies.

Handbook of Neuroemergency Clinical Trials
Publisher:Academic Press | Language: English | ISBN:0126480826 | 339 pages | Data: 2005 | PDF | 4 Mb
Description: During the 1990s, scientific advances in understanding the mechanisms and pathophysiology of acute central nervous system injury were offset by a history of disappointing results from Phase III clinical trials of novel neuroprotective drugs. Numerous novel compounds were tested, and seemingly fell by the wayside. This book is intended to focus on novel therapies and the unique challenges their intended targets pose for the design and analysis of clinical trials.
he authors explore the issues facing research in this area and the strategies that might lead to future success in this critical area of unmet medical need. It represents a compendium of information gained from over 20 years of clinical trial experience in areas of acute neurology and neurosurgery. From the knowledge of clinical assessment using standardized tools, to the intricate design of difficult hyper-acute neuroemergencies trials, the reader will benefit from the authors perspectives.
